In addition to this form, please submit all of the information electronically including either formal architectural plans (or drawn plans), the plot layout and drawing. Please do not fail to notate all setbacks. Attach  the informational pages that address the following or note if an item is not applicable to your situation / project.

I. Plot Plan

A. Detailing block and lot numbers on the plan						
B. Lot dimensions with North direction shown						
C. Building and setback dimensions						
D. Driveway location and dimensions						
E. Madison Addition plot layout with your lot circled						
F.* Total square footage (or %) of lot occupied by improvements
G.* Driveway material (must be similar to existing required driveways)

II. Floor Plan - Required only for new construction and accessory buildings

If your lot belongs in a lot "A" and lot "B" configuration and you don’t own both lots "A" and "B", AND you share a common wall with the other adjoining "A" or "B" lot, please have your corresponding lot owner sign and date this form to show their agreement with this application proposal. If adjoining "A" or "B" lot homeowner does not agree with this proposal, he/she should submit their objection in writing to the Board and AC in order to have their objection taken into consideration. If the proposed residence is being built crossing over two lots,  the lots are required to be aggregated into one lot.

If you are a member of the Nez Perce Owners Association, check you NPOA covenants prior to submitting any application. As a member of legal liability, MAOA cannot enforce NPOA covenants through our Architectural Committee. We cannot even consider them in our decisions. However, if your lot belongs in a lot "A" and lot "B" configuration and you do not own both lots "A" and "B", please have your corresponding lot homeowner sign and date this form to show their agreement with this application proposal.

By submitting this form, I hereby agree that the final project will not differ from the enclosed plans, or I will submit addition al information to the committee for approval. I am advised that Sections 8.6, 8.7 and 8.8 all allow the Committee to demand the suspension of construction and to pursue the enforcement in Court against me if I fail to follow these plans. Also, that the Committee is entitled to recover it's Court and Attorney fees in any such suit. I have been advised that if alterations are going to be made to my project, I will submit and gain approval for any alterations before they are made in my construction. If I have any questions or concerns, I am advised to seek answers before proceeding.
